The Office Staff is available by phone Monday through Friday between 9:00 AM and 3:00 PM.  Calls received after hours will be returned the next business day.

The office will contact you within a few days of your appointment to schedule your follow up visit. You can call the office or request your appointment online. 

Please allow 24 - 72 hours for medication refills 

On-line Payments should now be made through PayWoot

Your statements have your Account Number and Form ID needed to set up your PayWoot account. (click here for more information)

Insurance updates need to be made before your upcoming appointment.

Verify your Telehealth coverage prior to your scheduled appointment.

The completed Record Release Authorization form will allow us to share information and/or communicate with the designated person/facility regarding the patient.  

  • The form should be completed in full and signed by the patient (or legal representative) giving permission.  

  • Multiple releases can be completed to allow communication with family, guardian, doctors, etc.
