It is your responsibility to notify us of changes to your insurance

and provide copies of updated Insurance Cards Prior to Each Appointment

Call the office to update your information over the phone or use the Submit Updated Insurance Form.  We will also need a copy of the front and back of your insurance card.  You can send copies of insurance cards via:

​​Please be sure to verify your Behavioral Health Benefits with your insurance plan.  Failure to do so may leave you financially responsible for unexpected insurance deductible expenses or denied claims  due to not having out of network coverage.

Please check with your insurance plan to obtain your individual coverage and applicable deductible. We are not In Network with all plans, so please be aware of your Out of Network coverage


**When checking if our providers are In Network with your plan, please verify the Location is "Life Management for Adults" as our providers are also In Network with some insurance plans at Portsmouth Hospital.  If our provider is showing In Network with your plan at Portsmouth Hospital and NOT at Life Management For Adults then you will not be covered to be seen at this practice.**
